FAS3140 Boot Issue - unexpected mlog termination

I have an old FAS3140 connected to a DS14MK2, I'd like to boot it into maintenance mode to wipe the whole thing and reinstall DOT 7-mode but when I start up the FAS3140 I see the following errors:

Is it possible to do a factory reset w/o being able to boot into the boot menu?

Try NetBoot and install new software first.

I tried your suggestion and it still showing the same error msg.  Afterwards I went to the extent of re-creating the CF card and still shows the same errors:

Given there is no root vol for this host yet I'm confused as to where it is trying to access '/etc/nsswitch.conf' and '/var/log/mlogd-start.log' from.

Probably environment is messed up. Could you try printenv in loader to record current values, set-defaults and printenv again so it can be compared. Then try once more.
