how do i find out from the oncommand or command prompt that it did fail? what can i do?

You can look through the messages log file(s) for the failure. In this case, since the error was from last week, check the messages.0 file.
Or if you want to see the current status of the disk, from cli you can do a disk show command, like so:
SAN-1NETAPP04> disk show 5a.00.6

vol status -f
and replace the drive, call netapp open up a support case to send the drive and they will replace for you.
