Packing slip for FAS9000/AFF A700A

Hi all,
Does anyone have some packing slips for FAS9000A and AFF A700A ?
I would like to know following part numbers are on the slip or not;

   FAS9000A-001
   FAS9000A-002
   AFF-A700A-001
   AFF-A700A-002

If possible, I would like to have photos of the slip for FAS9000A and/or AFF A700A.
