Trying to setup SQL Server Monitoring, but keep getting invalid credential. What are the credentials needed for this monitoring? SQL DB account? Domain Account? What roles and permissions are needed for this monitoring?

Steven,

     In order to map a Microsoft SQL database back to the storage, you do need SQL level credentials.  Everything OnCommand Balance does is read only; however, at minimum it requires public and db_datareader permissions on each database.  Take a look at this knowledge base article: https://kb.netapp.com/support/index?page=content&id=1012899&actp=search&viewlocale=en_US&searchid=1374820984928
