Was the VM hardware version upgraded?  This same behaviour will occur starting around vm hardware version 11.

Drive A is missing.  Add a floppy drive to the VM, and connect a blank floppy image.  Then see if it will come up.

The fat16 volume is expected to be on disk1s2.
